The Health: Beauty category is sites listed by topic, where beauty is a category, you will note it is not for sites selling beauty products, only for primarily informational sites.
Regional: North America: United States: Alabama: Health is a category where the primary concern is location (and at state level for information about health only within Alabama and Health facilities in multiple Counties in Alabama).
I seriously doubt there will ever be an informational site about 'beauty' that is limited to being valid only in Alabama. What there are is sites that sell goods only within Alabama, and those sites would be listed in a Regional/North_America/United_States/Alabama/Business_and_Economy/Beauty_and_Cosmetic_Services/ if there were enaough to create the category.
If that category existed, it would be linked from within the Alabama/Health sub category with the name "Beauty and Cosmetic Services@".

Sites in Regional are for information or product sales limited by area (in this case State).
If you are looking for a category for a site that sells beauty products remotely (online, mail order, phone order, etc) and delivers to most of the US or internationally then somewhere in http://www.dmoz.org/Shopping/Health/ .
Nevertheless if a site only belongs in Regional it will not be listed in a Topical category merely because the exact sub category does not (yet) exist in Regional.
In this case if Regional/North_America/United_States/Alabama/Business_and_Economy/Beauty_and_Cosmetic_Services/ does not yet exist , then the site should be suggested to Regional/North_America/United_States/Alabama/Business_and_Economy/
