I have tried 5 times (8-23-02, 12-5-02, 1-13-03, 3-5-03, 5-18-03) to submit the following. I would greatly appreciate if anyone can advise of the status, errors I have made, or if should I re-apply. The last one on 5-18-03 received a Server Error, and I was not sure if it went or not, but if it did please use the following info instead as it was incorrect. I had inadvertantly used a to a deeplink.
