Where is this submission form ?

I keep following the directions and click "Suggest URL" and keep getting looped back to the help files. I cant find the the form link ?

Please give the specific category as a clickable link starting http://dmoz.org/

There are over 500,000 categories, most do work, and it helps to know which ones may not.
